To make your first visit as smooth as possible, we ask all new clients to complete the required forms at least 24 hours before their initial appointment. This allows our providers to engage more fully with you and/or your child during your visit.
After you have spoken with one of our team, you will receive a secure link to access, complete, and submit the forms prior to your first appointment.
