Schedule a follow-up appointment

Keeping regular follow-up appointments is an important part of your care. Follow-ups allow us to check in on your progress, make adjustments to the treatment plan, and ensure medications remain safe and effective. Most follow-ups are scheduled every 1-3 months for established clients, depending on your needs.

Medication refill requests

Our goal is to ensure your treatment is both safe and effective. Medication refills are only provided when clients are actively engaged in care and up to date with follow-up appointments. Please make every effort to keep your scheduled visits, as this is essential for monitoring your progress and adjusting treatment if needed.

If you are running low on medication before your next scheduled appointment, you may submit a Refill Request by messaging your provider through your patient portal.

Please note:

  • Requests are reviewed during regular business hours only.

  • Refills are not guaranteed and will be provided at the discretion of your prescribing provider.

  • A short-term or “bridge” refill may be approved to allow time to schedule a follow-up visit.

  • One emergency or bridge refill per calendar year may be provided, accompanied by a $50 administrative fee (as outlined in our Financial Policy).

  • Frequent last-minute or urgent refill requests may delay processing and could impact continuity of care.

Safety Reminders

  • Refills cannot be issued for patients who have not been seen within the recommended follow-up interval.

  • Lost or stolen prescriptions, early refill requests, or controlled medication renewals will be handled in accordance with federal and state regulations and the provider's discretion.

  • Please allow up to 2 business days for refill requests to be processed.

How to Request a Refill

  1. Confirm that your next follow-up appointment is scheduled.

  2. Click below to access your patient portal and send your provider a direct message.

  3. Check your patient portal or pharmacy for confirmation once the refill has been processed.

We encourage you to request refills during your scheduled visits whenever possible.
