Through this portal, you can:
Complete and sign your intake forms before your first visit
Book and manage appointments
Upload identification and insurance information securely
Send and receive messages with your provider
View visit summaries, treatment plans, and document
Getting Started
Once your initial appointment is scheduled, our team will email you a personalized link to activate your Patient Portal account.
After logging in, please complete the following steps at least 48 hours prior to your first visit to ensure a smooth registration and evaluation process.
1. Under the “Forms” tab:
Complete and electronically sign all required intake forms.
Using a device with a camera, upload clear photos of the front and back of your photo ID and insurance card.
2. Under the “Profile” tab:
Verify that your personal information, address, and insurance details are correct.
Complete all patient history sections as thoroughly as possible — the more detail you provide, the more time your provider can dedicate to discussing your evaluation, diagnosis, and treatment plan.
